To be able to participate in Invictus Soccer Performance's sessions, please read and sign the following form.
I hereby grant permission to Invictus Performance LLC to use my likeness, voice, and performance as recorded on audio or video tape without payment or any other consideration. I understand that my image or voice may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.
I hereby release Invictus Performance LLC its agents, employees, and assigns from any and all claims, demands, and liabilities whatsoever in connection with the use of my image, voice, or performance, including, but not limited to, any claims for defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright.
I understand that Invictus Performance LLC is not obligated to use my image or recording in any way. I acknowledge that I have read and understand this waiver and release, and that no oral representations, statements, or inducements apart from this written agreement have been made.
